

Bridging the Gap is a dedicated professional support person that deals with logistics related to in home care.
Bridging the Gap moves with agility between the client, family, and involved third parties, to ensure a smooth transition between all health-related activities. It provides information and support that make the shifts between these activities as seamless as possible. Various ‘’care kits’’ and templates prove useful when setting up a long- or short-term care environment and helpful project management and compassion are applied to overseeing logistics and seeking solutions to often unforeseen challenges that arise in a care setting.
Over the past four years, Pauline has gained insights into what it means to be a primary carer. To manage the balancing act of being both administrative support as well as emotional support to people who need a backup person in their lives during a health crisis and for life admin. Anyone who has been in a similar situation can relate to how alienating the realms between medical fields, home care and all the supplies needed to keep a care environment going, can be. For those who have never experienced this, it can be daunting and overwhelming. Pauline created Bridging the Gap to address this need.
There is often stigma attached to frail care, care environments and palliative care since it brings us face to face with our own mortality. These environments can often be challenging to spend time in, especially if you are well and enjoying life. Pauline has experienced these issues but employs techniques around practising kindness, empathy and humility that keep her focused on providing the care and looking after herself in the process.
Asking for help, support and planning can be tremendously burdensome in difficult circumstances. Having systems and plans in place generally and for emergencies are the foundation for creating a safe and secure care environment. Involving a third party can help lessen the load and redirect the burden to professionals to manage. Finding this support can often be challenging and Pauline is able to share her knowledge in this realm. Pauline is a ‘’millennial’’ so she can easily navigate communication relying on practical technology where required, to ensure information is organised in an accessible way.
Knowing firsthand how alienating, isolating and administratively overwhelming looking after others can be, Pauline has embarked on an innovative journey to bridge the gap between care related administration, logistics and planning, all in the form of a compassionate and intuitive professional support person.
Bridging the Gap brings together all of Pauline’s skills as a strategic thinker, project manager, trainee community arts counsellor and visual artist – enabling her to be a resilient critical thinker, with a creative flair for problem solving and emotional support during turbulent times. Pauline offers solutions and systems that give the client a foundation to design their own care plans, as well as provides a wealth of local information about how to make the care plan a reality.

One of our core offerings is creating digitised and organised photographic collections, visual memoirs/scrapbooks from late estates or for clients wishing to preserve memories, developing digital filing systems that preserve legacy and honour family narratives.
This includes:
– Archival Management & Digitisation: photos, images transformed in digital files and archives according to client needs.
– Visual Memoirs / Special Scrapbooking of precious memories/ mementos and documents. Preservation of personal/family legacy materials and storytelling.

Pauline Borton started Bridging the Gap after several years of informal primary care work in a palliative care setting.
Her interpersonal skills and many years of working with diverse communities enable her to be agile, pro -active and resilient in the face of crisis and make her an ideal support person in the most trying of circumstances.
Pauline does not believe that anyone should have to deal with health challenges alone, and that loved ones who may be affected by these circumstances should be choosing where their energy goes, not snowed under with planning and logistics.
To address this, she has set up supporting structures inspired by nature’s best use of resources and energy and is able to apply these to everyday life with empathy, ease, and pragmatism

Bridging the Gap offers a bespoke personalised service for each client and/or family, and to maintain this level of service there is a cap on the number of clients we can work with at a time.
